Withdraw from the MMT 100

If you need to withdraw from the 2012 MMT, please use the form below. If you withdraw on or before April 16, we will send you a refund less $25. If you withdraw after April 16 but by May 1, we will send you a refund less $50, but only if there is someone on the wait list to replace you.

By using this form, you will help us keep track of removing you from the entrants list and sending you a refund.
